a) Have you had symptoms, (Fever/chills, body aches, headache, fatigue, cough, shortness of breath/ difficulty breathing, runny nose/ congestion, loss of taste/ smell, sore throat, diarrhea, or nausea/vomiting) in the last 2 weeks?
b) Have you or someone you‘ve been in contact with have Covid or suspected Covid in the last 2 weeks?
c) Are you awaiting test results due to symptoms?

Note: We have Medify air purifiers with H13 or HEPA 13 filters throughout the office.
